Home visits:

We offer home visits for the the Village member and for those who choose to have their healthcare in the convenience of their own home.

We also offer virtual/phone/email visits or communication for the established Village community member that meets criteria for this type of visit. We also offer same-day visits.
IV Infusions

Almost any visit can be done in the comfort of your own home

Home Visit - Additional $40 (May be completely covered by insurance)

We would be happy to see any patient in their home. Please call for trip charge pricing.  

Telemedicine

Virtual visits with a provider via phone and/or video. 

New Patients

For patients that are new to the Village

• Comprehensive Exam
  Comprehensive intake, review of your medications, health history, complete physical, to include recommended labs and assessments for a client that is new to the Village community
  
• Urgent Visit
  A focused visit for injury or illness that cannot wait until a scheduled comprehensive exam for the client that is new to the Village community 

Established Patients

For existing Village community members

• Comprehensive Exam
  Comprehensive intake, review of your medications, health history, complete physical, to include recommended labs and assessments for an existing member of the Village community
  
• Routine Visit
Problem-focused visit for routine health maintenance of chronic conditions and medication to include a physical examination and recommended labs and assessments for an existing member of the Village community
Urgent Visit
A focused visit for injury or illness that cannot wait until a scheduled comprehensive exam for an existing member of the Village community 

Pediatrics

We see ages 10 and up

• Comprehensive Exam
  Comprehensive intake, review of any medications, health history, complete physical, and developmental assessment to include recommended labs and assessments for a child that is new to the Village community
  
• Urgent Visit - new patient
  A focused visit for injury or illness that cannot wait until the next recommended visit for children that are not currently members of the Village community
  
• Urgent Visit - established patient
  A focused visit for injury or illness that cannot wait until the next recommended visit for children that are members of the Village community 

Laboratory Testing at Reasonable Rates

We can provide laboratory collection and results, with review and recommendations, at rates that are much lower than most hospitals and clinics when they are routine. Urgent/STAT labs will be billed directly to the Village member/insurance. 

This service is also available for established home & facility patients.

Radiology and Diagnostic Testing

We can offer basic radiology exams in the clinic and portable X-ray, ultrasound, ECHO, EKG in the home setting. These services will be billed directly to the Village member/insurance. 

This service is also available for established home & facility patients.

B12 injection package pricing

New Village membership discount - talk to your provider or staff member to see if you qualify 

B12 Injections

Individual injection - $25 (1000mcg of Vitamin B12)

Packages:

• "The test drive" - 3 monthly injections ($90)
• "Beat the blues" - 6 monthly injections ($150)
• "Best year ever" - 12 monthly injections ($240) 

Mental Health Treatment

We treat general mental health issues such as anxiety, depression, bipolar disorder, and age-related mental health issues. We also offer urgent virtual appointments to help our patients get the timely care they need in the comfort of their own home when appropriate

Payment Options

1. We will work to accept your personal insurance (We are in-network with most commercial and state insurances)
2. We are willing to work to accept additional coverages
3. We accept Medicare/Medicaid
4. We offer completely transparent and affordable fee-for-service options (Cash/Credit)
5. We also will work out payment plans for those who do not have any of the above for necessary medical care
